HCBA Coach Ejection/Incident Form

This report is to be completed and submitted within 24 hours of any ejection of any coach or player in a Hamilton Cardinals Rep Baseball Association game. A Game includes any pre-season or exhibition, regular season, tournament (in Canada or International) and any OBA Championship or National tournament or elimination game. This form is also used for all major injury report for any person involved in a Hamilton Cardinals activity (game, practice, event etc.) The head coach of the Hamilton Cardinals team must send in reports.
